Original location of the Sanctuary: Until the 1930s, hundreds of various types of ex-votos (wooden tablets, canvases, goldsmith objects, prostheses) were placed on the walls and hung from the vault of the building. Notes on the collection: In 1927, during a pastoral visit, Bishop Pagano ordered the ex-votos to be concentrated in the oratory of the confraternities; in 1931, Bishop Macchi had the objects removed, which remained disorderly on the walls due to the resistance of some faithful, and ordered to accept only silver hearts. Following this measure, a large part of the votive objects, attested between the 17th and 19th centuries, were lost. Typology of ex-votos: Painted tablets, Goldsmith objects, Real or represented prostheses Current preservation: Currently, some silver hearts and four 19th-century canvases (1838-1898), which escaped the dispersals of the early century, are displayed along the walls of the church. Reference to publications or printed descriptions: A description of the ex-votos preserved at the sanctuary in the second half of the 17th century is given by Macolino, Historia della Miracolosa Apparizione, 1686.
The first historians of Gallivaggio report, in addition to subsequent miraculous events, the detailed list of the first twenty-nine miracles performed by the Virgin: Chiaverini, Breve narratione delle prerogative, 1663; Chiaverini, Historia della miracolosa Apparitione, 1667; Macolino, Historia della Miracolosa Apparitione, 1686.
The origin of the sanctuary is linked to the apparition of the Madonna, which occurred on the morning of October 10, 1492, to two young girls who were gathering chestnuts in the San Giacomo Valley, Gallivaggio locality. After inquiring about the living conditions of the two girls, the Virgin Mary called for the conversion of sinners and pointed to some recent events (a comet that appeared the previous year, an earthquake, a violent storm that hit the Grisons) as signs of the imminent divine wrath, from which humanity would escape only through conversion. Finally, the Madonna emphasized the need to observe Sunday holidays starting from 3 PM on Saturday.
The oldest account of the apparition, probably dating back to the end of the 15th century and now lost, was transcribed by Gian Giacomo Macalino in the work on the sanctuary published in 1686 (Macalino, Historia della Miracolosa Apparitione, 1686)). Seventeenth-century copies of the same account are preserved in the historical archive of the Diocese of Como and in the Laurenzian Chapter Archive of Chiavenna (see entry Sources).

Currently, the spiritual care of the sanctuary is entrusted to a religious of the Servants of the Immaculate Heart of Mary.
Since the first attestation regarding spiritual care (1515), the sanctuary has been one of the seven vice-cures of Valle San Giacomo, administered by a rector elected by the faithful. Over the centuries, the rectors of Gallivaggio were mostly seculars originating from Valchiavenna, sometimes, especially in the 19th century, already holders of parishes in the area.
From the first attestation (1515) to the entire 18th century, the patronage over the sanctuary was exercised by the community dependent on the vice-cure of Gallivaggio.
