    In 1884, a restoration was undertaken, entrusted to engineer Pio Casoli: eclectic in style, only the bell tower remains from the original (1884). Description: a small wooden tablet with a painting of the Madonna and Child, commissioned immediately after the miracle by the miracle recipient himself and initially hung on the tree where the saving apparition occurred. The elm tree is also considered an object of worship, as the sanctuary was built around the plant. It was destroyed along with the aforementioned tablet during the looting by the troops of Ottavio Farnese in 1557.     The sanctuary is still operational. Its activity was interrupted between 1805 and 1843 (converted into a barracks until 1832, then into a hospital); and during World War I. It is a diocesan sanctuary.

    From 1859, spiritual care passed to the parish priest of San Donnino. In 1887, a community of Mantellate Servants of Mary was established at the sanctuary, living monastically in cloister but being the actual custodians of the sanctuary.
    The congregation (Redemptorist Fathers) maintained spiritual care until 1859, with an interruption in the years 1848-1850.
    Between 1797 and 1843, the church continued to be officiated.
    The spiritual care of the Servants of Mary ended in 1797, with the arrival of French troops.
