Composed of two rectangular-plan bodies connected by a central octagonal body. It ends with a lantern on the roof of the octagon.

This date (1639) is indicated by two scholars (E. Bernardi and P. Giacobazzi). According to others, the date should be placed around 1630, based on two pastoral visits (in 1632 and 1629) that would indicate the temple not only built but also functioning. Its fame (of the sanctuary) is nonetheless due to the establishment in 1644 of the Confraternity of Consolation and the Belt, now transformed into the Confraternity of the Blessed Sacrament, which, under the impetus of parish priest Don Bartolomeo Ferrari, formed and took under its protection the Montesino oratory and took care of its maintenance. Towards the end of the 18th century and the beginning of the 19th century, the sanctuary went through a period of crisis coinciding with the crisis faced by the confraternity to which it had been entrusted: the assets and members underwent a sharp reduction. Only around 1840 was there a clear recovery.
In 1772, the assets of the oratory were enfeoffed under the Malaspina della Rosa family, and from here began the period of decline that ended about fifty years later. Among the main benefactors of the oratory, we remember Domenica Passini, whose donation around the end of the 17th century had allowed the construction of a hermitage. The date indicated for the entry into force of the first parish is that related to the oldest document preserved in it.
