The history of the Shrine has its roots in a distant past. According to scholar Ansano Fabbi, the original building was a Farfa monastic cell, dedicated to St. John the Baptist. In 1231, it passed to the Abbey of Ferentillo and, subsequently, in 1303, it became part of the Lateran Chapter by order of Boniface VIII, following the abolition of the abbey. A history of transitions and transformations that shaped its identity.

The decisive turning point occurred in 1477, when the church was rebuilt and assumed the title of Madonna Apparente, or Madonna Appare, or Maria dell’Apparizione. A Marian apparition is recounted, although there are no oral traditions to confirm it, which gave the shrine its name and consecrated its role as a place of worship dedicated to the Virgin. Since then, the building has become a singular example of a votive church, connected to an oratory by a suggestive archway under which the road still runs today.
