The church features a single nave structure topped by a gabled roof. A tower bell tower, elegantly detached from the main body of the building, completes its architecture. The heart of the sanctuary is the painting depicting the Madonna with the sleeping Child. An image of tenderness and protection, enriched by profound symbolism. Behind the Child, a monstrance, symbol of the Eucharist, can be seen, while on the sides, Saint Matthew, Saint Luke, and the allegorical figures of the angel and the ox, traditionally associated with the two evangelists, are recognizable.

In 2000, the church was awarded the title of jubilee church, a significant recognition on the occasion of the Great Jubilee.

Tangible evidence of the faith and gratitude of the faithful are the ex votos preserved in the sanctuary. Painted tablets, goldsmith objects, and other votive gifts tell stories of received graces and miracles attributed to the intercession of the Madonna del Piastraio.

The painted tablets, created by Guglielmo Tommasi himself, offer a glimpse into the daily life and sufferings of the local people, a historical and artistic document of great value. For security reasons, the current location of the ex votos is not disclosed, but their existence testifies to the deep popular devotion and the indissoluble bond between the community and its sanctuary.
