On July 23 of the same year, among the ruins of the ancient oratory, Mercorella found a wooden painting depicting the Madonna, with the exact same features as in his vision. This miraculous event marked the beginning of a new era of Marian devotion at the site.

The current building of the Sanctuary, the result of the 17th-century reconstruction, features a three-nave Latin cross structure, typical of the religious architecture of the time. Inside, the sacred image of the Madonna del Terzito is preserved, a painting of great artistic and spiritual value.
