The first documented mention of the sanctuary dates to the Pastoral Visit of Monsignor Oliva on May 14, 1568, which cites the church of “Sanctae Mariae del Carmino” and its rector, Don Giovanni Antonio del Sangro. This confirms the existence of a place of worship dedicated to Madonna del Carmine as early as the 16th century.

Over the centuries, the sanctuary underwent various transformations and challenges. In 1866, the convent and sanctuary of the Observant Friars Minor were suppressed and acquired by the State, which transferred them to the municipality of Palmoli in 1869. In 1937, the Congregation of the Sons of the Holy Family purchased the entire complex from the municipality, restoring a portion that had fallen into ruin. This intervention safeguarded the sanctuary and preserved its historical and religious value.
